Immunohistochemical Scoring of C4d in Egyptian Patients with Chronic Renal Allograft Dysfunction

چکیده
C4d is a degradation product of complement component C4. Since the beginning of the current century, C4d has been used as a marker of acute antibody mediated rejection in renal allograft and considered as an ominous portent for poor graft survival. The ninth Banff meeting emphasized the value of C4d in renal allograft biopsy assessment and adopted a grading system for the staining degree of C4d (0 through 3). The twelfth Banff meeting revised the proposed grading system. The literature data about the role of C4d in chronic renal allograft damage is variable. We conducted a retrospective study for cases with biopsy proven chronic renal allograft damage diagnosed at our nephropathology units, between Jan. 2007 and Sep. 2013 in order to detect and score C4d positive cases comparing the variability between C4d scores in correlation with the available clinical data and histopathological parameters of chronicity. Twenty eight cases (54.9%) were C4d positive. The frequency of cases with scored positive staining was as follows: C4d1 (12 cases, 23.5%), C4d2 (2 cases, 3.9%) and C4d3 (14 cases, 27.5%). No significant clinical or chronic histopathological findings between score (1) and score (3).

Capillary deposition of complement split product C4d in renal allografts is associated with basement membrane injury in peritubular and glomerular capillaries: a contribution of humoral immunity to chronic allograft rejection.
Endothelial deposition of the complement split product C4d is an established marker of antibody-mediated acute renal allograft rejection. A contribution of alloantibody-dependent immune reactions to chronic rejection is under discussion. In this study, the association of immunohistochemically detected endothelial C4d deposition in peritubular capillaries (PTC) with morphologic features of chron...

Background: Regulatory T cells have been suggested to have a protective role against acute rejection in allograft recipients. However, there is little information available about their contribution to chronic rejection process. The role of transforming growth factor-beta 1 (TGF- β1) as a profibrogenic and/or immunoregulatory cytokine in renal allografts is also controversial. Objectives: To eva...
متن کاملRenal graft biopsy assists diagnosis and treatment of renal allograft dysfunction after kidney transplantation: a report of 106 cases.
Acute antibody mediated rejection (AMR) is one of the most important complications after kidney transplantation. Renal graft biopsy is safe and reliable without adverse effects on the patients and transplanted kidneys, which was of great instructive significance in diagnosis and treatment of renal allograft dysfunction after renal transplantation. This paper reported a case series of 106 patien...

متن کاملCombination of peritubular c4d and transplant glomerulopathy predicts late renal allograft failure.
The histologic associations and clinical implications of peritubular capillary C4d staining from long-term renal allografts are unknown. We identified 99 renal transplant patients who underwent an allograft biopsy for renal dysfunction at least 10 yr after transplantation, 25 of whom were C4d-positive and 74 of whom were C4d-negative. The average time of the index biopsy from transplantation wa...
